Endometriosis excision surgery is the technique by which the disease is removed by cutting it out. In short, excision allows for complete removal of endometriosis lesions, as such the recurrence of the disease is prevented and symptoms are improved.

How does excision surgery works?

Endometriosis excision refers to the removal of endometriosis tissues while preserving the uterus, tubes, and ovaries. In order to better understand how excision works, there is an analogy between endometriosis and iceberg. Same as the iceberg, in endometriosis lesions only the top part is visible, while the deep part of the lesion is still within the underlying tissue. When the whole part is removed,  there is no residual lesion present. This has very low recurrence rates in the long-term, symptom relief and improves fertility in patients wishing to conceive.

When operating on endometriosis patients it is also important to preserve healthy tissue function.  Surgery is the first and best management for the diagnosis and excision of endometriosis. Endometriosis excision surgery should always be performed by an endometriosis specialist and a multidisciplinary team for the best results. Endometriosis affects most often the peritoneum, which is a thin membrane that covers the visceral organs. Excision allows this layer to be peeled away, and so the endometriosis tissue can be removed. Deep endometriosis tissue requires more removal, and in order to achieve a complete resection of the tissue, the normal healthy tissue must be identified from the abnormal tissue. This can be achieved by using excision.

Although endometriosis might recur, excision reduces the recurrence rate to 15-20%. Excision also provides the greatest benefit for patients with extensive disease without increasing the rate of complications or morbidity. Moreover, excision is the best method to use in patients wishing to preserve fertility.
